- NOC Letter with company stamp
- Notarized Original Invitation Letter, Guarantee Letter and a Business Registration copy from Company in Korea.
- Original Passport, valid for more than 6 months and has more than two blank visa pages left, and a passport copy(A-4 size).
- Copy of the travel itinerary such as flight schedule or hotel information
- Evidence of intended medical treatment from medical institution in Korea
- Recent original stamped bank statements
